Position - Care Co-Ordinator
Job Description
We are looking to recruit two dedicated Care Coordinators to work closely within the Care team to ensure Service User care packages are appropriately staffed and managed.

Salary: £14.68 per hour PLUS On Call at £12.71 per hour ,
1x Fixed- Term contract : 12 months ( maternity leave cover) . The Care Coordinator will be contracted 30 hours and will work approximately 16 hours additional On Call per week (this will vary from week to week). Daily working hours: Monday, Tuesday, Wednesday & Friday, 9.00am to 5.00pm.
1x Part-Time contract, permanent : The Care Coordinator will be contracted 22.5 hours and will work approximately 12 hours additional On Call per week (this will vary from week to week). Daily working hours: Monday, Thursday & Friday, 9.00am to 5.00pm.
Purpose
The role of Care Coordinator will be to schedule and co-ordinate our carers’ weekly rotas using an electronic rostering system. You will be in the hub of all activity, ensuring effective communication between Management, Health and social care professionals, the Carers and the service users.
Job Responsibilities
- Working within a very friendly team, you'll be based in the office and be responsible for scheduling the weekly care workers rotas and to assist in the day-to-day coordination of the care hours including arranging for emergency cover as and when required.
- Liaise with Health and Social Care professionals on matters relating to the Care package and service delivery
- Work in a calm and organised manner in a fast-paced environment
- Ensure holiday, sickness and emergency care calls are assigned appropriately and efficiently.
- Arrange and conduct Risk Assessments and Home Care Assessments of service users to identify their care needs.
- To carry out regular monitoring of service provision to ensure that quality standards are being met
- Visit Service Users and make reviews of the written Care Plans, examine the written records of medication administration, finance and daily log notes kept by staff.
- Carry out and attend regular team meetings with Care Staff
- Carry out spot checks and Supervision/Appraisal reviews of the staff’s performance.
- Verify that RQIA minimum standards and DHSSPS regulations are adhered to at all times
- You will be expected to cover calls in emergency situations to ensure continued care
- You will have the on-call phone on a planned schedule
- To assist with induction, training and supervision of staff
- To maintain accurate records and provide written reports on complaints, accidents and incidents
Essential
- Experience working on database systems and knowledge of Microsoft Office
- Full driving Licence and access to a car for work purposes
Desirable
- NVQ Level 2 or equivalent (in Health & Social Care)
- At least 1 year experience working in the care sector
- Professional qualification/training
- Experience as a supervisor or senior care worker or other role within a care providing organisation
- Proven ability to prioritise and work towards deadlines.
- Compassion and understanding of the challenges of the care sector.
- Have a calm and polite telephone manner.
- Excellent organisational and problem solving skills
- Ability to work under pressure
